- Customers must present their identity cards and print the order confirmation letter on the appointment day.
- The health check service purchased is valid for two months (from the date of purchase) or it will be forfeited.
- The SARS-CoV-2 IgG II (Quantitative) Antibody Test purchased is valid for three (3) months (from the appointment date of Pre-vaccination Protection Check-up Plan) or it will be forfeited. The date of perform the Antibody Test needs to present the valid invoice which collected after the body check plan, photocopies or damaged invoice will not be accepted.
- The Antibody Test is required to take the test at least 2 weeks after the completion of 2 doses of COVID-19 vaccination.
