COMMON QUESTIONS
Being a small and flat butterfly-shaped organ located in the front of the trachea in the neck, the thyroid gland is difficult to observe and feel under normal circumstances. It is responsible for producing and releasing thyroid hormones, which are essential for regulating metabolism and growth and development in the body.
Hyperthyroidism (overactive thyroid): Hyperthyroidism is caused by the thyroid gland overproducing thyroid hormones. This can be caused by the following:
- Autoimmune disease: Graves’ disease is the most common autoimmune hyperthyroidism disorder. The immune system mistakes thyroid cells for foreign invaders, causing the thyroid to overproduce thyroid hormones.
- Thyroid tumors: Some thyroid tumors can cause excessive thyroid hormone secretion, leading to hyperthyroidism symptoms.
- Excessive iodine intake: Consuming too much iodine, such as through excessive intake of iodine-containing foods or medications, can stimulate the thyroid to overproduce thyroid hormones.
Hypothyroidism (underactive thyroid): Hypothyroidism is caused by the thyroid gland’s inability to produce enough thyroid hormones. Causes of hypothyroidism include:
- Autoimmune disease: Chronic lymphocytic thyroiditis (Hashimoto’s thyroiditis) is the most common autoimmune hypothyroidism disorder. In this condition, the immune system attacks the thyroid, damaging thyroid cells and making them unable to produce thyroid hormones normally.
- Thyroid surgery: If a patient undergoes thyroid surgery or radiation therapy, thyroid tissue may be removed or damaged, leading to hypothyroidism.
- Iodine deficiency: The thyroid needs iodine to synthesize thyroid hormones, and if the body is deficient in iodine, the thyroid cannot function properly, leading to hypothyroidism.
Common symptoms of hyperthyroidism and hypothyroidism
Hyperthyroidism (overactive thyroid) patients commonly experience symptoms such as rapid heartbeat (palpitations), weight loss, increased appetite, hand tremors, sweating, mental agitation, insomnia, diarrhea, bulging eyes, and changes in mood and temperament.
Hypothyroidism (underactive thyroid) patients commonly experience symptoms such as slow heartbeat, weight gain, slow reactions, depression, fatigue, weakness, constipation, and difficulty concentrating.
Thyroid self-examination
In addition to regular physical exams, you can also perform a thyroid self-examination. Generally speaking, the most direct way is to look in a mirror and observe your neck for any lumps or swelling. In addition, you can gently feel your neck with your hands to see if there are any abnormalities.
Thyroid tumors can cause goiters
The “goiters” we often hear about can be caused by thyroid tumors, benign thyroid hyperplasia, adenomatous hyperplasia, or cysts. Dr. Kwong pointed out that thyroid tumors are very common, and when the thyroid is enlarged, if the thyroid tumor is found to be malignant, the tumor obstructs the esophagus or trachea, hyperthyroidism (hyperthyroidism) and other symptoms, then consider surgical removal of the thyroid.
The thyroid absorbs iodine in the body and converts it into T3 (triiodothyronine) and T4 (thyroxine), which are used to regulate metabolism, growth, blood pressure, and heartbeat. As for how much thyroxine the thyroid secretes, it depends on the levels of T3 and T4 in the blood: when T3 and T4 levels are low, the pituitary gland secretes “thyroid-stimulating hormone” (TSH), which stimulates the thyroid to secrete more T3 and T4; Conversely, when T3 and T4 are excessive, TSH secretion is suppressed.
Therefore, blood tests are preferable, which can be used to test the secretion levels of TSH, T3, and T4, and to perform thyroid ultrasound scans and related hormone levels. During the ultrasound examination, the doctor will use an ultrasound device to observe the size and shape of the thyroid. Some patients may need to undergo biopsy tissue examination to rule out malignant lesions. If necessary, they can be referred to an endocrinologist and diabetologist for follow-up.
The treatment for hyperthyroidism depends on the patient’s age, physical condition, cause, and severity. Treatment options include:
Antithyroid drugs – Because they are convenient to take, they are often used as the first-line drug for treating hyperthyroidism and can stop the secretion of thyroid hormones and gradually reduce symptoms. Patients usually need to receive 18 to 24 months of treatment, and most patients relapse after stopping the drug.
Radioactive iodine treatment – Patients who do not respond to antithyroid drugs or who choose this treatment method based on personal preference. Radioactive iodine is taken orally, absorbed by the blood vessels and transmitted to the thyroid, and can eliminate part or all of the thyroid to permanently reduce the activity of the thyroid. Patients may need to take medication for life to maintain normal hormone levels.

- Urine test: Collect the middle portion of 15ml urine stream in the clean container before the test.
- Stool test: Stool size should be walnut or 1-inch ball in the clean container within 24 hours before the test or can get the special stool container in our testing centre. (It is important that no urine or toilet water mix with the stool sample).
- Abstinence for at least 24 hours before the test is recommended. Avoid cleaning up your genital in the test day morning.
- Avoid visiting the bathroom 2 hours before the test.
- Ladies in menstruation is not suitable for urine test.
- Urine Test: Recommended to collect the sample of your first morning urine on the day.
- Pelvis U/S Scan: Have full bladder prior to examination.
- Please inform our staff of confirmed/ suspected pregnancy.
